Admission & immunization
GCC’s Health Services Office closed effective August 24, 2012 and will no longer offer clinical services. The Health Records office is located in the North Building on the ground floor in Room G07 and is responsible for reviewing general immunization records, and processing health forms for admission into our health career programs. If dropping off forms when the office is closed, please leave them in the Admission Office instead. They should be in a sealed envelope to protect your confidentiality.
The main phone number to the Health Records office is 413-775-1430. Questions regarding general immunization records should be directed to Donna Larson RNC, 775-1432 or larson@gcc.mass.edu. Questions regarding health career medical records should be directed to Annette Moise RN, 775-1431 or moisea@gcc.mass.edu. If you are seeking a medical withdrawal, forms are available and should be dropped off in the Registrar’s Office. Medical withdrawal reviews will be done by the Registrar.
In compliance with the Commonwealth’s College Immunization Law, all full-time students (enrolled in 12 or more credits) and all full-time and part-time international students must provide the Health Services Office with documentation of immunity to specified communicable diseases within 30 days of starting classes. Failure to comply with these requirements will jeopardize continued enrollment, receipt of grade reports, transcripts, etc. Medical and religious exemptions are allowed.

Health requirements for admission to Health Occupations programs – In addition to proof of immunization, all full- and part-time Health Occupations students must provide the Health Services Office with documentation of health history and a physical exam. Health Occupations students will not be permitted to attend classes without a completed health record.
